AMENDED TRACK AND TRACE PROCEDURE

On each separate occasion you come to the Club, either at weekends or during the week, for whatever reason you should now comply with the Track and Trace procedure by:

EITHER:

Scanning the NHS QR code displayed on the Clubhouse window to the left of the Clubhouse front door, or displayed on the Committee Room door (the first door on your right as you enter the Clubhouse), using your NHS Covid-19 App on your mobile phone. You will first need to download the NHS Covid-19 App from the App Store or Google Playstore onto your mobile phone.

BY SENDING AN EMAIL not more than 2 days in advance to:

in the following manner:

In the TITLE of the email put your own name AND the date of attendance in dd/mm/yyyy format.
In the BODY of the email put the following information:

• The times of day as accurately as possible you expect to be at the Club, from and to
• The names of ALL persons accompanying you
• A contact telephone number

These details will be held for 21 days as per Government requirements to assist in tracking any infection.
In the event someone tests positive for Covid-19 but you are unwilling for us to inform the Government Tracking Service you were present at the Club, please advise us of this in the email you send.
